Please book online as a “New Patient” via Health Engine, alternatively please call our friendly Reception team on (08) 9751 4500 for assistance.
If you have booked your appointment online, you will receive a link to our electronic new patient form to complete prior to your appointment.
if you have booked over the phone, please attend your appointment 15 minutes early to fill out the new patient paperwork, OR print and fill it out (link below) and bring to your appointment.
