A few simple steps to register and prepare for your first appointment at Sunshine Medical Centre.

Register through Healthengine, or call reception on (08) 9751 4500 to begin.

Complete your form
You'll receive a link to complete our electronic new patient form before your visit.

Or arrive early
If you can't complete the form online, please arrive 10 minutes early to complete a paper form.

Bring your details
Bring your Medicare card, any concession cards, a list of current medications and previous specialist letters or test results.
Transferring your medical records
We can request a copy of your medical records from your previous GP on your behalf. This requires your written consent and signature — our reception team will provide a transfer request form when you register, or you can complete one at your first appointment.
Cancellation policy
We understand plans change. To be fair to other patients waiting for an appointment, a $60 fee applies for cancellations with less than 2 hours' notice or non-attendance. Please call us as early as possible if you need to reschedule.
